Honestly I can't stand this kid, but I can see he has a real mental disorder. Now there's nothing wrong with having a psychological disease, being born with one is not their fault. It's obvious to me this kid is not taking care of himself, and THAT is his fault. Imagine a person with diabetes not taking their insulin, it's dangerous. Bipolar is no different, just dangerous in different ways. There are lots of people who have mental disorders who live a perfectly normal life. Bipolar is very manageable, there's lots of different treatments too. No one else but himself can seek them out.

My suggestion on how to handle this kid. Ignore him, don't fight with him under any circumstances. All this kid wants is a fight and to feel powerful since he feels so weak inside. Don't let him think he is powerful, or he will keep going.
