Go into advance and should be a box you gotta check to add poll. Then it will ask how many lines, put 2. Then it will take you to another page, put title as Perma Ban, then yes and then No. All done.
I know I added the demo to Sourcebans but in the future add the demo to either Sourcebans or to the Perma Ban Thread.
