Go into advance and should be a box you gotta check to add poll. Then it will ask how many lines, put 2. Then it will take you to another page, put title as Perma Ban, then yes and then No. All done.
I know I added the demo to Sourcebans but in the future add the demo to either Sourcebans or to the Perma Ban Thread.

correction, if you go to the top right corner you'll see an option that says thread tools and click on it and in the drop down menu select add a poll and it'll then take you to the add a poll screen

edit: obvious aimbot, as soon as you get a poll added we can vote on it unless a clan member can take care of this
