Reason for perma ban: Speedhacking
I, and the other players didn't realize he was speed hacking till near the end of the map when he was running away from a zombie. When he was called out, he really pulled on a good show. When he was called out, I spectated him, didn't see much at the time, then round ended. He left before next round could start, I banned him for 30 days from the disconnected player ban list, pulled up the video and have some tick marks available.
7500 - Swimming/shooting in water, moving rapidly sideways/forwards.
9000 - Running faster towards the bridge with the box in the middle, (caught up with his teammate who was clearly ahead of him)
20000 - End of the round, again he speeds up to catch up to a teammate at the nuke button.
23000 - As a zombie, he quickly tags two people at spawn
25000 - As a zombie and no humans around watching him, he really lets his speed hacks go
65000 - Not relevant to speed hack, but stayed back at spawn intent to kill first zombie (well not really intent to kill, but threw nade at them)
68000 - Running away from zombie as human, gets called out around 69000 tick mark.
72000 - Does not give a shit that he's speed hacking, he lets it go.
As an admin, sorry I didn't see this for a whole map, but as some people who do play with me know, I sometimes play css while I'm at work, so sometimes I am an AFK Spectator working on cars.
Hope this is enough for a perma ban.
