The revive is based on a % and that gets reset when you get revived. for you Brett if you raised yourself that many times you had a 0.04308042% chance of that happening when maxed, just consider yourself lucky.

As for the nebula thing, I'm going to say, teammate is just meaning "someone on the team you're on, self inclusive" or "member of the CT or T team, whichever you're on".
